ACTIVE SLR TEST FOR INSTABILITY OF SPINE

How to perform?
Active SLR test is perfomed with the patient in supine position and both his hands behind the head. Then he is asked to lift both the legs at the same tie without bending the knees.The bakpain reproduced by this maneuvre is taken as apositive test.
What does it show?
It indicates the instability phase of the degenerative spine disease with weak abdominal muscles .
Mechniasm:
Lifting of legs stright---->rotation of pelvis anteriorly---->hyperextension of spine due to weak abdominal muscles--->increased intradiscal pressure------>reproduction of the backpain----->positive test.
Implications:
* pt has weak abdominal muscles and during conservative therapy, the pt. should strngthen the abdominal muscles and wear a corset that raises the intra-abdominal pressure and supports the spine better.
*Pt should avoid extension strains on spine and follow a flexion exercise programme .
*If the instability indicated by the test does not respond to the conservative therapy, then the surgery involves fusion of that motion segment.
